A comprehensive onboarding roadmap for transition-age youth, social workers, and placing agencies. Everything you need for a smooth, dignified move-in.

Placement & Confirmation

Referral reviewed, intake completed, and bed reservation locked within 24–48 hours by our placement desk.

Day 1 Move-In & Orientation

Welcome tour, room keys, welcome hygiene pack, and initial meeting with your assigned Case Manager.

First 30 Days & Plan

Creation of your Individualized Transition Plan (ITP), enrollment support (education/jobs), and life-skills goals.

Preparation

Move-In Day Checklist

All homes provide fully furnished single-occupancy rooms with fresh bedding, towels, and closet storage.

What to Bring

  • Identification: State ID/Driver's License, Social Security Card, and Birth Certificate (if available).
  • Clothing: 7–10 days of casual, school, or interview attire. Laundry machines & detergent are on-site.
  • Personal Care: Favorite personal hygiene products (welcome basics provided upon arrival).
  • Medications: Current prescriptions in original pharmacy containers with doctor labels.
  • Electronics: Phone, charger, laptop or tablet for school and employment search.

What to Leave Behind

  • No Weapons or Firearms: Zero-tolerance policy for weapons, knives, or dangerous items.
  • No Unprescribed Substances: Alcohol, cannabis, or illicit substances are prohibited on premises.
  • No Large Furniture: Rooms are fully furnished (bed, desk, chair, wardrobe, nightstand).
  • No Pets: Service animals approved via intake documentation prior to arrival only.
  • No Open Flame Items: Candles, incense, and hot plates are prohibited for fire safety.
Community Living

House Agreements & Expectations

Our standards are designed to protect resident safety, cultivate mutual respect, and create a peaceful environment for personal growth.

🕒 Curfew & Wellness Check

Standard evening curfew (10:00 p.m. weeknights / 11:00 p.m. weekends) with flexible passes for work or academic commitments approved by Case Management.

🤝 Weekly Case Management

Mandatory weekly 1-on-1 check-ins with your assigned Social Worker to track life skills, savings goals, employment milestones, and permanent housing search.

🏡 Shared Chores & Cleanliness

Residents maintain their private room and rotate light communal chores (kitchen wipe-down, trash removal) to maintain household dignity.

🛡️ Privacy & Single Occupancy

Every resident is entitled to privacy in their single room. Staff respect private space with advance notice for scheduled inspections.

🚫 Overnight Guest Policy

For safety and licensing compliance, no unapproved outside visitors or overnight guests are permitted inside the residence.

🎓 Work or Education Active

Residents actively engage in school, vocational training, internship, or employment search with staff support and guidance.

County Notice & Transition Guidelines

Second Chance Homes operates under strict compliance with California Community Care Licensing (CCL) and LA County DCFS/Probation transitional housing standards.

Protocol 01

14-Day Transition Planning

Before any resident moves out or graduates into permanent housing, a mandatory 14-day transition planning conference is held with the youth, case manager, and county social worker.

Protocol 02

30-Day Written Notice

Standard programmatic discharges or non-emergency transitions require a minimum 30-day written notification to the placing county agency before room reassignment occurs.

Protocol 03

Emergency Safety Notice

In immediate safety situations, immediate verbal notification within 2 hours is provided to the designated caseworker, followed by written incident documentation within 24 hours.

Note for Placing Agencies: Room availability signals on this platform reflect authorized bed vacancies. Rooms with pending 14-day or 30-day transitions are marked as "Limited / In Transition" until official county clearance and move-out inspection are completed.
